As you probably know by now, Open Enrollment for dental insurance (and to make any changes to your current medical insurance) is going on from now through November 30. The Benefits Office has sent out emails to all employees with instructions.

There was a preliminary letter sent to all employees that may have been a bit misleading or, at best, easily misunderstood. Please note that your response to the preliminary letter did not enroll you in the dental plan. You must complete an Open Enrollment Change Form between now and November 30 to be enrolled for dental coverage.

The Benefits Office sent emails to this effect, but just in case you missed it, you can find the form online right here and send it to the Benefits Office by school mail.

Fill out the top part, check "Active Employee", enter any changes to your medical coverage or leave blank if you have no changes. To enroll only yourself for the free basic dental, simply check "Add", "Change to Basic", enter your name, "self" under Relationship and Date of Birth. Sign and date at the bottom of the form.

Note: Please notify the SNEA office - snea@aol.com - if you want to cancel your SNEA/AFLAC Dental Plan. The Benefits Office cannot do this for you.
